From the beginning of the season, 42-year-old Kim had concerns about how her 44-year-old sister Kourtney would react to her role as the creative director for a Dolce & Gabbana fashion show in Milan. This concern stemmed from Dolce & Gabbana personally styling Kourtney and Travis Barker's wedding. After the fashion show concluded and the family returned home, Kourtney, who made headlines with her recent gender reveal party, opened up about what truly bothers her about her sister Kim, who was born just 18 months after her.

The episode begins with Kim Kardashian taking her glam squad to a Los Angeles DMV for her driver's license photo. The scene then transitions to Kourtney's home when Khloe arrives. Khloe enters the house and notices Simon Huck, who informs them that they are setting up for Kourtney Kardashian's vitamin and supplement brand. He compliments Khloe's appearance, saying she looks "gorgeous." Simon asks if Kourtney has spoken to Kim yet, to which Khloe responds, "No, and that's why I'm here. I feel like... I mean... is that her behind me?" as Kourtney appears to greet Khloe.

Simon inquires if Khloe will be attending the Baby 2 Baby gala, to which Khloe confirms but mentions she hasn't found an outfit yet. She adds, "So, I'm potentially going. As of now, I don't have an outfit." Simon mentions that he is coincidentally attending and sitting at the Dolce table. Khloe responds with, "Speaking of Dolce..." In her confessional, Khloe comments, "Whatever is going on with Kourtney and Kim, the tension is brewing and brewing, and the more time that goes by, the thicker the tension gets. Kourtney interjects, "If she felt bad, don't you think she would reach out and say, 'Hey, I was thinking about it and I really feel blah blah blah. Like I don't think that she feels..." Khloe interrupts, "To my understanding, she's tried to talk to you, but you haven't been ready to talk." Kourtney corrects her, "Zero percent false... I mean, 100 percent false, zero percent true."

Kourtney admits, "It comes from an old thing of us being 18 months apart and just having a competitive nature as sisters. Then I think the one time I've even slightly done anything that was similar to her, which was like hang some Christmas lights in my entry that were like similar, which I didn't even see hers at the time, like didn't even know. Didn't even know and it was such a thing."

She continues, "I feel like I'd go to big extents to have my own thing, but there's like no sense of loyalty or feeling of like, oh, how would I feel? We have everything, more than everything that we want or need. If I look at the photos from the fashion show, like, half the time I'm like, is this my wedding? Like?"

Khloe asks, "But why can't you say that to her?" Kourtney responds, "Because I'm also like it's who she is to her core." In her confessional, Kourtney explains, "This is definitely not about a brand. It was about my wedding. I didn't feel truly supported. I think it's just that it's like a free-for-all. It's like no boundaries, and it felt like there's no decency to ask me how I felt about her doing this so closely to my wedding. It's also like no one just gives a f**k about anything. It's like the Wild West, like we're just gonna take and do as we want, whatever is best for us."

Back at Kourtney's house, Khloe asks, "So what are you gonna do? Not talk to her forever?" Kourtney responds with a laugh, "That's an option." Khloe suggests, "Let's just go through our options here. Have the talk soon, have it in six months when you implode, or what? With the amount of energy you're giving to him and me, why don't you just give it to her and let this be done?" Kourtney explains, "Cause she's so intolerable to even have a conversation with. She's like on her phone the whole time, can hardly look up, really hard to engage with."

Khloe proposes, "What if we put the phones down for the talk?" Kourtney adds, "It just makes me want to run the other way. I just thought, what else can you take from me? Can I have anything that's mine?" In her confessional, Kourtney reflects, "Our family is so interconnected, and we always have been. All of our businesses were all together. But I think, for my survival, I need my own identity, and I need just my own life. I really love having some separation and having my own friends and having my own family."

Khloe recalls, "I never knew there was a problem until I spoke to you, and you were crying. That was the first time I knew there was an issue. This was in Milan. She called just to say hi to me and to check on me and then just started hysterically crying, rightfully so, but I had not a clue there was an issue." Kourtney adds, "I think one reason we, as a family, have been able to not be complete diva psychopaths is that we have each other to check each other."
